ABSTRACT

Although the surgical treatment of colorectal disease progresses slowly, with the advancement of minimally invasive surgical techniques, perioperative comprehensive treatment strategies and clinical research in recent years, Chinese colorectal surgery has developed rapidly. Transanal total mesorectal excision, lateral pelvic lymph node dissection,"watch and wait"strategy for clinical complete response of neoadjuvant radiotherapy for rectal cancer, and robotic colorectal surgery are still hot topics in colorectal surgery in recent years. The construction of clinical registry database and clinical research for colorectal cancer surgery are discussed, with a view to combing the development of colorectal surgery for colleagues in the surgical field, and to seek the development of colorectal surgery in China.

ABSTRACT

China has the largest group of patients with colorectal cancer in the world, however, there are few international guidelines for the diagnosis and treatment of colorectal cancer from China, or research of high-level evidence in China. In recent years, colorectal surgery has developed rapidly in China, and great progress has been made both in clinical practice and in clinical research. But there are still many imbalances and under-regulations. In 2018, Chinese Society of Colorectal Surgery led the establishment of Chinese Colorectal Cancer Surgery Database (CCCD). The retrospective and prospective data collection coming from CCCD showed that the standardization of colorectal cancer surgery and data management needed to be improved, and the awareness of clinical research needed to be further improved. Therefore, how to carry out good clinical research and obtain high-level evidence based upon CCCD will be the great challenge for colorectal surgery in China.

ABSTRACT

Transanal total mesorectal excision (TaTME) is a new technique emerging in the field of colorectal surgery in recent years.It can solve some difficult problems of traditional open and laparoscopic surgery,and achieve good short-term clinical results.It is becoming more and more popular among colorectal surgeons in China.In the world,TaTME-related database construction,structured training,and clinical research are almost synchronized with its clinical practice.How to analyze the problems existing in the clinical practice of TaTME operation nationwide,improve the safety and effectiveness of the technology,guide its structured training,and clinical research,and build and maintain the nationwide TaTME registry database is a very effective way,but it needs a strong organizational and management framework.
